- One staff did not have ELCCT. This staff is no longer employed. Three staff did not have CPR & FA. Staff received cpr & FA One staff did not have Safe Sleep training. Staff received safe sleep training. One staff received professional development training hours. No equipment inventory was located. Three staff did not have personnel information form sent to licensing. Personnel information forms were emailed.
