Non Compliance Observed Two staff members were hired in June 2025 and personnel forms have not been submitted to Licensing. Numerous, Repeated, and/or Serious Non Compliance No Numerous: No Repeated: No Serious Non Compliance: No Plan to Correct Personnel forms were obtained by Licensing while Licensing was on site. Program director will follow the New Hire Checklist and will make sure the send personnel forms as they are hired. Regulation Description submitted to Licensing within two weeks of employment. Infection control
Non Compliance Observed All staff members except for one are past due to complete annual infection control. Use zones
Non Compliance Observed CCLS observed 3 swing sets in the outdoor play area that did not have use zones.
