Self Reported incident of 2 children leaving outdoor playground due to a gate not being shut, this being due to lack of supervision.

Director states children were pickup and brought back into outdoor play area. Director states in the future that they will have floating teacher come out to playground prior to each class coming out and do a playground check to make sure gates are secured & closed and there are no hazards for the children. Also all personnel will make sure before coming back into building that all gates are secure and closed. During personnel meetings it will be stressed the importance of supervision of the children must be done at all times. Also personnel involved will be written up for the incident. In the future Director will make sure all gates are tightly closed so that they will remain in compliance.

The program is required to maintain supervision at all times.
