Non Compliance Observed Licensing observed diaper changing pad in infant room to be torn and needs to be replaced. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Director stated she will purchase a new changing pad. In the future, the diaper changing pad will be replaced when torn. Non Compliance Observed Licensing observed refrigerator and freezer to be unsanitary with dried food particles. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Director stated she will clean the fridge and freezer today. Non Compliance Observed Licensing observed pea gravel on playground needs to be raked and leveled for use zones. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Director stated she will rake the gravel and pull it in from the edges. In the future the pea will be raked and leveled for use zones. Regulation Description are maintained at a depth of at least six inches by replacing, leveling, or raking.
