Non Compliance Observed Outside playground had wood rot leading to a splinter hazard and iron rods sticking up. Outside playground also had equipment with sharp edges that was a potential hazard. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Provider will replace all wood border to remove hazards, and provider has removed the equipment with sharp edges during visit. Non Compliance Observed Program had multi-axis swing that did not meet the 30 inch clearance to the fence. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Provider will remove the multi axis swing from the play area. Regulation Description for multi-axis swings, such as tire swings with circular motion, extend six feet plus the length of the chain in every direction from the midpoint.
