Three children's Insurance Notifications over 12 months old, 1 child's Insurance Notification was not signed/dated by parent & 1 child did not have an Insurance Notification form. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Ms. Scroggins will have the parents complete new forms today. Ms. Scroggins will email/text pictures of each child's forms to Licensing staff. Provider will set a reminder to complete new forms one month prior to when they're due. Regulation Description Parents complete OKDHS Forms 07LC045E, Insurance Notification, and 07LC046E, Compliance File Notification, every 12 months. Forms are maintained at the facility.
