Non Compliance Observed Licensing observed that the canine's vaccinations expired on 5/15/2025. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Provider states that she contacted the vet and made an appointment for 6/28/2025 will get dog 3 year vaccinations to ensure that she is covered for the next few years. Regulation Description Current rabies vaccination administered by a licensed veterinarian for dogs and cats is documented and maintained on the premises. Plan to Correct Date 2025-06-30
Place to rest
Non Compliance Observed Three children were observed laying down in mini-trampolines with a mat on top of the trampoline bottom where children would jump. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct This was corrected during the visit when the children were removed from the inappropriate rest equipment. Provider ensures that children will not sleep in trampolines again. Regulation Description Each child has an appropriately sized, individual place to rest, such as a crib, playpen, play yard, bed, cot, or mat, with clean, individual bedding. The place to rest is maintained in a clean, sanitary condition, and in good repair. Plan to Correct Date 2025-06-24
Mats
Non Compliance Observed Licensing observed a soft cushion-like material was wrapped in a tarp & duct-taped together. There was wear on several which made them not waterproof. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Provider states that she plans to purchase either mats or cots and ensures that she will purchase enough for the program. Regulation Description Mats. Mats are not used for overnight care. When used for rest time, each mat is at least one inch thick and covered with a fitted, durable, washable, waterproof material.
