Non Compliance Observed Licensing observed children's records to be missing insurance notification forms. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Licensing provided Primary Caregiver six copies of the Insurance Notification form. Primary Caregiver will have each present child's parent fill out and sign the form upon pick up. In the future, insurance notification forms will be provided to parents and signed annually. Regulation Description Parents complete OKDHS Forms 07LC045E, Insurance Notification, and 07LC046E, Compliance File Notification, every 12 months. Forms are maintained at the facility.
