Non Compliance Observed Licensing observed program to not have maintained child information forms for 2 children enrolled. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Provider will handout child information forms to parents today, and have them bring them back in the morning. Provider will ensure prior to enrollment she obtains a child information form for each child enrolled. Provider will maintain them on file at all times. Parent notifications
Non Compliance Observed Licensing observed program to not have maintained parent notifications for 2 children enrolled. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Provider will have parents sign the parent notification forms at pick up today, and will ensure she obtains the notification forms upon enrollment and every 12 months thereafter. Provider will maintain them with child files at all times. Regulation Description Parents complete OKDHS Forms 07LC045E, Insurance Notification, and 07LC046E, Compliance File Notification, every 12 months. Forms are maintained at the facility. Media use
Non Compliance Observed When Licensing arrived it was observed that the television was playing in room with children that was not age-appropriate. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Provider corrected this at time of visit. Provider will ensure when children are in the same room with TV on, that age appropriate shows are shown and that they do not go over 30 minutes a day.
