Non Compliance Observed Licensing observed a 1 yr old and a 3 yr old being transported facing forward in the vehicle on site at the time of visit. Numerous, Repeated, and/or Serious Non Compliance Yes Plan to Correct Primary caregiver will place the car seats in the correct position rear facing and will ensure all children are in the child restraints correctly. Regulation Description Passenger restraints and seating. Children remain properly secured in a child passenger restraint system, such as car or booster seat or individual seat belt, per Section 11-1112 of Title 47 of the Oklahoma Statutes (47 O.S. § 11-1112). Vehicle requirements
Non Compliance Observed Licensing observed additional items required for vehicle first aid not verified on site at the time of visit. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Primary caregiver will restock both the program and vehicle first aid kits and will ensure that when items are used that they are replaced. Primary caregiver will check monthly to ensure that the first aid kits are always stocked with required items. Professional development requirements
Non Compliance Observed Licensing observed the Personnel job positions are not listed correctly on the Direct Care Organization, therefore the program is not maintaining all items listed on the Direct Care Facility Handbook. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Primary caregiver will correct the positions on the account and will remove those not associated with the program. The primary caregiver will ensure that the DCO account is kept current. Professional development requirements
Non Compliance Observed Licensing observed one assistant caregiver not having completed 12-clock hours of professional development annually on site at the time of visit.
