Non Compliance Observed Serious Licensing observed a hunting arrow, a folding knife, and a fixed blade knife accessible in a room being used for children. Numerous, Repeated, and/or Serious Non Compliance Yes Plan to Correct Licensing had primary caregiver remove weapons and store according to licenses. Primary Caregiver will go through the bedroom and clean it out to make room is hazard free. In the future will maintain requirements. Regulation Description Weapons. Parent notifications
Non Compliance Observed Licensing observed that program did not have on file current compliance file notification for six (6) children in care did not and current insurance notification for four (4) children in care. Numerous, Repeated, and/or Serious Non Compliance Yes Plan to Correct Primary Caregiver will have parents fill and sign forms needed for the children. Primary Caregiver will choose a month that paper work will be updated yearly. In the future will maintain requirements. Non Compliance Observed Licensing observed that the program had two (2) children in care without immunization record or exemption on file. Per Oklahoma Administrative Code (OAC) 340 Appendix II – Immunizations, an immunization record or exemption is obtained prior to the first day of attendance and is updated when the child receives additional vaccines. Non Compliance Observed Serious Licensing observed an infant asleep on the coach with a couch pillow beside the infant and the infant was covered with a blanket. Numerous, Repeated, and/or Serious Non Compliance Yes Plan to Correct Infant was Assistant Caregiver's baby. Licensing informed Primary Caregiver that all infants must follow rest time requirements. Infant was removed from couch. In the future will maintain requirements. Regulation Description Crib, port-a-crib, playpen, and play yard safety features and sleep environments. Smoke detectors
