Non Compliance Observed A live roach was observed in the office area. a few dead roaches were observed in the bathroom. Numerous, Repeated, and/or Serious Non Compliance No Numerous: No Repeated: No Serious Non Compliance: No Plan to Correct Primary Caregiver has had exterminators out once per week to treat for pests. This has been ongoing for 3 weeks. Primary Caregiver will continue to have home treated to rid of pests. Primary Caregiver will remove dead pests as needed. Regulation Description Maintenance. The home is in a good state of repair, clean and sanitary and has operable utilities. Plan to Correct Date 2025-12-16
