Non Compliance Observed Licensing observed provider to not have obtained and maintained one child's immunization record prior to the first day of attendance at time of visit. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Provider called parent at time of visit and the immunization record is going to be sent over to provider today. Once provider obtains the immunization record, she will let licensing know. Provider will ensure that prior to any child's first day of attendance she obtains a copy of a current immunization record. Regulation Description Immunizations. Per Oklahoma Administrative Code (OAC) 340 Appendix II – Immunizations, an immunization record or exemption is obtained prior to the first day of attendance and is updated when the child receives additional vaccines.
