Non Compliance Observed Licensing observed primary caregiver to not be maintaining attendance on self indicating days and hours worked on file at program. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Provider will start writing her own attendance on a calendar daily. Provider will ensure she maintains her own attendance daily indicating days and hours worked. Non Compliance Observed Licensing observed program to not have maintained current compliance notification forms every 12 months. Numerous, Repeated, and/or Serious Non Compliance No Plan to Correct Provider stated she will send out new packets to families by the end of this week. Provider will ensure she maintains current compliance file notification forms every 12 months.
